pharmachologic effect

Pharmacodynamics

The anti-inflammatory and anti-bronchoconstrictor activity of fenspiride is due to a decrease in the production of a number of biologically active substances (cytokines (especially TNFα), arachidonic acid derivatives, free radicals), which play an important role in the development of inflammation and bronchospasm.

Inhibition of arachidonic acid metabolism by fenspiride is potentiated by its H 1 -antihistamine action, tk. histamine stimulates the metabolism of arachidonic acid with the formation of prostaglandins and leukotrienes. Fenspiride blocks α-adrenergic receptors, the stimulation of which is accompanied by an increase in the secretion of bronchial glands. Thus, fenspiride reduces the effect of a number of factors that contribute to the hypersecretion of pro-inflammatory factors, the development of inflammation and bronchial obstruction.

Fenspiride also has an antispasmodic effect.

Pharmacokinetics

Overdose

In case of an overdose of the drug (cases of overdose have been noted when taking the drug at a dose of more than 2320 mg), you should immediately seek medical help.

Symptoms: drowsiness or agitation, nausea, sinus tachycardia.

Treatment: gastric lavage, ECG monitoring, maintenance of vital body functions.

drug interaction

Special studies on the interaction of fenspiride with other drugs have not been conducted.

Due to the possible sedative effect when taking histamine H1 receptor blockers, the use of Erespal in combination with drugs that have a sedative effect, or in conjunction with alcohol, is not recommended.

special instructions

For treatment children and adolescents under the age of 18 Erespal syrup should be used.

Influence on the ability to drive vehicles and control mechanisms

Studies to study the effect of the drug Erespal on the ability to drive vehicles and work with mechanisms have not been conducted. Patients should be aware of the possible development of drowsiness when taking Erespal, especially at the beginning of therapy or when combined with alcohol intake, and should be careful when driving vehicles and performing work that requires a high speed of psychomotor reactions.

Pregnancy and lactation

There are no or limited data on the use of Erespal in pregnant women. The use of the drug during the time is not recommended.

Colds are perhaps the most common among the population. During the year, they can overtake repeatedly, be accompanied by various symptoms and complications, so treatment should not be postponed. In this case, Erespal comes to the rescue - a syrup that doctors often prescribe, both for young patients and adults.

The pharmacy range of drugs prescribed for acute respiratory infections is very wide. Relatively new and already proven effective among them is "Erespal" - a drug produced in tablets or syrup.

Fenspiride, which is part of Erespal, is the main active substance, thanks to which the medicine shows its complex medicinal properties. Tablets and syrup have slightly different composition, however, common pharmacological effects.

But the liquid form contains a number of advantages over the tablet:

  • rapid absorption in the gastrointestinal tract;
  • faster result;
  • ease of use and dosing, which is important if the drug is prescribed for children;
  • pleasant taste.

Erespal in syrup is available in a plastic container with a lid with a capacity of 150 ml. Outwardly - an orange-colored liquid with a slight vanilla aroma. Before using the syrup, it is recommended to shake it well, since the sediment at the bottom is a variant of the norm.

Pharmacological properties and pharmacokinetics

Fenspiride has a complex effect on the body:

  • relieves inflammation;
  • exhibits anti-exudative action;
  • does not allow the development of bronchospasm;
  • inhibits the action of histamine receptors, reducing allergic manifestations.

In the gastrointestinal tract, Erespal is absorbed quite well, reaching a maximum concentration after 6 hours, but reducing it by a factor of 12. It is excreted from the body naturally - with urine and through the intestines.

What kind of cough is Erespal syrup prescribed for?

Cough itself is not a disease! And the patient should be treated not for cough, but for the disease of which he is a symptom. Coughing is often a reflex to clear the airways. With acute respiratory infections with lesions of the upper and lower respiratory tract, it is the cough that helps to free them from sputum.

Erespal is prescribed for such diagnoses as:

  • sinusitis (bacterial and viral);
  • pharyngitis;
  • laryngitis.

Syrup intake cannot be a complete replacement for antibiotic therapy. Therefore, Erespal is used as a component of complex treatment.

The use of fenspiride for otitis media, rhinitis (including those of allergic origin) and other ENT diseases, both accompanied and not accompanied by cough, is justified.

The anti-inflammatory effect of Erespal syrup leads to a decrease in exudation, removal of edema of the bronchial mucosa, and liquefies sputum. The effectiveness of Erespal has been clinically proven in both dry and wet cough, which occurs against the background of inflammatory respiratory diseases.

As a cough syrup, Erespal can be prescribed for whooping cough. This dangerous disease is most common among children under 2 years of age. Whooping cough is caused by special aerobic bacteria, while a person does not have innate immunity to them. And even vaccination, which significantly reduces the risk of infection, does not provide complete protection against whooping cough. Erespal in this case does not relieve the disease, but significantly alleviates the patient's condition, reducing the severity of a severe and debilitating cough. Bronchial asthma is another disease in which the drug can be prescribed. A characteristic symptom of asthma is bronchial obstruction - a narrowing of their lumen, which is manifested by shortness of breath and coughing. Due to the action of fenspiride, the lumen of the bronchi increases, the risk of mucus stagnation decreases. Therefore, it is advisable to use Erespal for bronchitis, chronic pulmonary obstruction.

Instructions for use and dosage of the drug

Erespal tablets are not prescribed for children! Syrup, on the other hand, is prescribed to small patients in the exact dosage that must be observed. For adults, the liquid form of the drug is also possible for use.

Erespal syrup for children

Kids quite easily agree to be treated with Erespal due to its pleasant taste and smell. The smallest - up to two years, Erespal is not assigned at all. In other cases, the instructions for use regulate the following dosage: 4 mg of the drug for each kilogram of the baby's weight during the day, that is, the daily rate of the drug can be given in 2 doses. This is done before meals, the bottle is shaken well beforehand.

On average, it takes:

  • 10-20 ml (similar to 2-4 teaspoons) of syrup per day for children weighing up to 10 kg;
  • 30-60 ml (from 2 to 4 tablespoons) Erespal for children weighing more than 10 kg.

In the presence of certain indications, at the discretion of the attending physician, a different dosage is possible. The duration of use of the drug is also determined by a specialist.

Erespal for adults

Adult patients and children from 12 years of age take 3-6 tablespoons in a daily volume (which is equivalent to 50-90 ml). In terms of fenspiride, 1 tablespoon of syrup (15 ml) contains 10 mg of the active substance.

Can I use the drug during pregnancy and lactation

During pregnancy or breastfeeding a baby, medicines for a woman are selected very carefully. The weeks of the first trimester are considered especially dangerous, since at this time the fetal organs are laid. Chemical and even plant components of drugs can have a negative impact on this process. And during the period of breastfeeding, substances are able to penetrate into milk, causing allergies or problems with breathing, digestion and the work of the central nervous system in the baby.

Erespal syrup is not intended for use as a medicine for expectant and nursing mothers.

However, with caution in some cases, it can be prescribed if the benefits for the mother outweigh the negative impact on the child, and the attending physician adequately assesses the potential risk.

Drug interactions with other drugs

Special studies on the interaction of Erespal with other drugs have not been conducted. At the moment, no compatibility has been identified. There are suggestions that the combined use of fenspiride-containing and sedatives enhances the effect of the latter. It is not recommended to combine treatment and alcohol intake. A negative result of such a joint use may be a deterioration in well-being, weakness, lethargy and drowsiness.

Contraindications, side effects and overdose

Erespal syrup for children and adult patients is considered a safe and effective drug. However, it also has contraindications for use.

Among them:

  • intolerance to fenspiride and other components that make up the composition;
  • age of children up to 24 months;
  • diabetes mellitus: the drug contains sucrose - up to 0.9 bread units per tablespoon.

In addition, the medicine contains parabens in its composition. They are considered potential allergens, the effect of which can manifest itself only after a fairly long time.

The most common side effects from taking standard drugs are: difficulty in digestion, nausea, heart palpitations, drowsiness, or, conversely, sleep disturbance. An overdose of syrup is manifested by similar adverse reactions. If the prescribed rate of the drug is exceeded, the victim should wash the stomach, if you feel worse, call an ambulance or seek medical help yourself. Treatment is symptomatic, in some cases an ECG, drug treatment, and support for the vital functions of the body may be required.

Erespal syrup analogs

Erespal does not have an absolute analogue in terms of active ingredients. The syrup is produced by the French company Servier Laboratories, as well as the representative company in Russia, Pharmstandard-Leksredstv, in the city of Kursk. Erespal's analogues are selected by the doctor, taking into account the individual characteristics of the patient.

It can be:

  • Siresp is a syrup with fenspiride in the composition, which has anti-inflammatory and bronchodilator properties. It has the same contraindications. Dosages are similar, taking into account the age and weight of patients.
  • Sinekod (drops) is a strong drug that suppresses the activity of the cough receptor. The active substance of Sinekod is Butamirat, these drops can only be taken with a dry cough. Allowed for babies from 8 weeks of age.
  • Ascoril is a syrup that allows you to quickly alleviate the patient's condition with a debilitating cough, which has a complex therapeutic effect. It has a complex composition, where the active ingredients are guaifenesin, bromhexine and salbutamol. Ascoril is indicated for small patients from 6 years of age.
  • Ambrobene is a syrup containing Ambroxol, it effectively dilutes and removes sputum, promotes accelerated recovery from respiratory tract infections. Ambrobene is prescribed for children from 2 years of age.
  • Lazolvan is a syrup prescribed for coughing with sputum that is difficult to pass, which is based on ambroxol. Not suitable for oral use in children under 2 years of age.
  • Stoptussin-phyto is a cough syrup based on guaifenesin and butamirate. The drug simultaneously splits sputum and partially blocks the cough center, which is good for an unproductive dry cough.
  • Cashnol is a remedy in the form of a liquid syrup with the active ingredients salbutamol and bromhexine. Assign to patients from 6 years old as a mucolytic and expectorant.

Any remedy that is supposed to be given to a child for coughing should be discussed with the attending physician. In the absence of a visible effect when taking Erespal after a week of use, you should also contact a specialist and discuss further treatment.

Inflammatory changes in the respiratory mucosa are a fairly common occurrence among children of preschool and school age. Usually, this pathology occurs when various viral and bacterial pathogens enter the body. Therefore, the main task facing pediatricians is to strengthen local protective factors, reduce the manifestations of the mucosal inflammatory reaction, reduce the severity of cough and improve sputum drainage. All these effects are promised by the instructions for the use of "Erespal" for children.

An important advantage of this drug is that its anti-inflammatory effect differs from the effect of classical non-steroidal anti-inflammatory drugs or glucocorticoids. Therefore, "Erespal" is devoid of a number of side effects characteristic of these groups of drugs.

The action of the drug

The main active ingredient of "Erespal" is fenspiride hydrochloride. This substance is able to have a complex therapeutic effect on the structures of the respiratory system. Fenspiride has the following effects:

  • anti-inflammatory;
  • antiallergic;
  • stimulates the excretion of mucus.

The anti-inflammatory effect of fenspiride is explained by its ability to reduce the number of inflammatory molecules (prostaglandins, leukotrienes). The antiallergic effect is associated with an effect on specific receptors. Thus, the hyperreactivity of the bronchi decreases and their patency improves.

In what cases is it assigned

Fenspiride hydrochloride selectively affects the upper and lower parts of the respiratory system. Therefore, the range of application of this drug is quite wide. Indications for use are:

  • diseases of the upper respiratory tract (sinusitis, otitis media);
  • rhinopharyngitis, pharyngitis, adenoids;
  • laryngitis;
  • bronchitis, tracheobronchitis;
  • bronchoasthmatic syndrome;
  • dry or wet cough;
  • discomfort in the throat with viral infections (ARVI, influenza, measles).

"Erespal" can be included in a comprehensive treatment regimen for various respiratory pathologies. It is successfully used together with antibiotics, expectorants or inhalations. However, before using it, it is still recommended to consult a pediatrician.

Release form

The drug "Erespal" is available in the form of the following pharmaceutical forms:

  • syrup;
  • pills.

Syrup

Syrup "Erespal" is available in bottles containing 0.15 or 0.25 liters of medication. The content of fenspiride hydrochloride in it is 2 mg per milliliter of the drug. The syrup is a yellowish liquid with a sweet taste and a pleasant vanilla aroma. Children drink this medicine with pleasure, however, it should be remembered that there are additives in its composition. The components of the drug, in addition to fenspiride hydrochloride, are:

  • licorice extract;
  • vanilla flavor;
  • sucrose, saccharin;
  • glycerol.

Some components can cause allergic reactions in children predisposed to them. This point should be taken into account when Erespal is prescribed to a newborn or infant who has a tendency to allergies. The presence of sweeteners should be considered in the treatment of patients with diabetes mellitus. It must be remembered that five milliliters of syrup contains 0.3 XE (bread units).

Pills

Erespal tablets contain 0.08 g of fenspiride hydrochloride. They include a minimum number of additional components, however, their use is only allowed in patients who have reached the age of 18 years. Therefore, it is not recommended to give Erespal tablets to children under one year of age or preschool age, and if necessary, you should ask your doctor if this can be done.

How to apply

According to the official instructions, Erespal syrup is allowed to be given to children who have reached the age of two. However, in practice, Erespal is prescribed from the age of three months.

The drug is taken before meals, so its absorption in the gastrointestinal tract improves. Shake the medicine bottle well before use. For young children, syrup can be added to any drink or feeding bottle.
The required amount of "Erespal" is measured using the attached measuring spoon. Dosages are indicated in the table.

Table - Age dosages of "Erespal"

When calculating the dosage, it should be taken into account that one teaspoon contains 5 ml of syrup, and one tablespoon contains 15 ml. The dosage of "Erespal" for children under one year, as a rule, is calculated by the pediatrician individually, depending on the severity of the symptoms and the course of the disease.

Usually, the duration of the use of "Erespal" is seven to eight days, if necessary, therapy is extended to two weeks. How much exactly to give "Erespal" to a child, you should definitely check with your doctor.

Efficiency

According to pediatricians, the effect of the use of this drug is observed already on the second or third day of treatment. In children, nasal discharge, swelling and redness of the pharynx are significantly reduced, and coughing is reduced. Nevertheless, it is not recommended to use this medicine for the prevention of complications in acute respiratory viral infections.

Reviews of "Erespal" for children indicate that it is an effective drug that allows you to eliminate unpleasant symptoms in inflammatory processes in the respiratory tract.

The high efficiency of "Erespal" allows you to use it as a monotherapy. This eliminates the need to prescribe other drugs (anti-inflammatory, expectorant, antihistamines). Thus, the cost of treatment and the risk of side effects from various drugs are significantly reduced.

How is Erespal tolerated by children

Usually children tolerate Erespal well. Occasionally, when taking it, the following side effects occur:

  • nausea, rarely vomiting;
  • violation of the stool, pain in the abdomen;
  • dizziness, sometimes drowsiness;
  • increase in heart rate;
  • low blood pressure;
  • fatigue;
  • allergic reactions in the form of skin rashes, itching, rarely - Quincke's edema.

Extremely rarely, when taking large doses of the drug (more than 2.32 g), overdose phenomena occur. Its symptoms in a child are psychomotor agitation or, conversely, increased drowsiness, vomiting, diarrhea, and frequent pulse. There is no specific antidote for Erespal. Treatment is aimed at accelerating the elimination of the drug from the body and restoring vital functions.

The main contraindication for taking the medicine is individual intolerance to fenspiride hydrochloride and other components of the syrup. Also, the drug is used with caution in children with impaired metabolism of fructose and other sugars.

Acquisition and analogues

"Erespal" for a child refers to an over-the-counter dosage form. It can be freely bought without a doctor's prescription. However, before giving medicine to a baby, it is still recommended to consult a pediatrician about how often Erespal can be taken by a child.

The pharmacy chain also presents analogues of the original drug:

  • "Epistat";
  • "Siresp";
  • "Erisspirus".

These medicines also contain fenspiride hydrochloride as the main active ingredient. They are available in the form of a syrup and can be used to treat young children. The dosage of these drugs is calculated similarly to "Erespal", additional information is given in the instructions enclosed in the package.

The safety of Erespal has been proven in numerous clinical studies, so pediatricians are increasingly prescribing Erespal for infants and preschoolers if they have diseases of the respiratory system.
